Obverse: Shield of the Near East Relief organization. The shield includes several symbols: a crescent and star, a lamb carrying a flag, and a cross, representing unity, sacrifice, and Christian charity. Above the shield is a single star, and the surrounding inscription reads NEAR EAST RELIEF and OPVS IVSTITIAE PAX (Latin for "The work of justice is peace").

Reverse: Initials N.E.R. encircled by a laurel wreath, symbolizing honor and victory. At the top is the year 1915, and the surrounding inscription reads FOR FAITHFUL AND UNSELFISH SERVICE TO HUMANITY, acknowledging the contributions of those who aided in the humanitarian efforts.
